Quake prend place dans un monde où le gouvernement, après plusieurs années de recherche et d’expérimentation, a réussi à développer un prototype rendant possible la téléportation. Celui-ci est cependant corrompu par un ennemi, connu sous le nom de code Quake, qui utilise le prototype pour envoyer ses créatures cauchemardesque dans la dimension des humains, préparant ainsi une possible invasion de la terre. Le gouvernement décide alors d’envoyer une équipe de combattants à travers le portail afin d’éliminer cette menace.
Le joueur incarne un soldat anonyme, seul survivant de l’opération envoyée à travers le portail de téléportation pour éliminer le démon à l’origine des attaques contre la terre. À partir d’une base encore sécurisée, celui-ci peut accéder, via des portails magiques, à quatre dimensions contrôlées par l’ennemi : La Dimension of the Doomed, le Realm of Black Magic, le Netherworld et le Elder World.
